Accounts - User Name Change Request
The process of requesting a user name change for employee or student NetID and TitanApps accounts.
Username Changes are usually requested because of the following change
- The individual changes their first and/or last name
- The individual becomes staff after or while currently a student
Name change
For first and/or last name changes
Employees
- Employees must request their legal name change through the Human Resources Office first.
- Once the official name change has been processed by Human Resources then you can request a username change from Information Technology by calling the Information Technology Help Desk at 920-424-3020, putting in your own request at uwo.sh/service-portal, or emailing helpdesk@uwosh.edu.
Students
- Students must request their legal name change through the Registrars Office first.
- Once the official name change has been processed by the Registrars Office then they can request a username change from Information Technology by calling the Information Technology Help Desk at 920-424-3020, putting in your own request at uwo.sh/service-portal, or emailing helpdesk@uwosh.edu.

Contact the Information Technology Help Desk by calling the Information Technology Help Desk at 920-424-3020, putting in your own request at uwo.sh/service-portal, or emailing helpdesk@uwosh.edu.
and requesting a username change to a staff user name account.
Important Note: In order for Information Technology to put this username change request in, your hiring paperwork must be completed through Human Resources first.
